To schedule an appointment at Haluska Mental Wellness, LLC, please  click on the "Schedule Your Consultation" button on our home page to set up an account through our billing/credentialing company, Headway. Once that is done, we will reach out to get your appointment set up. 


Currently, we see ages 15 and up.  We request a free consultation for patients new to us to ensure we can safely provide care to patients.  If consultation determines a patient may require more than can be offered by our practice regardless of age, the patient/family will be notified and recommendations for higher level of care will be offered. 


No, we do not.


This is on a case-by-case basis. However, to consider filling out paperwork of any kind, patients will need to have an established relationship with the practice (at least 3 to 6 months depending on case) for us to be able to provide rationale required for much of this paperwork, and to be able to provide documentation that is often requested from companies. 


We do not prescribe benzodiazepines to be taken daily and we do not prescribe benzodiazepines if a patient is taking certain other central nervous system depressants (such as opioid pain medications or muscle relaxants).  If benzodiazepines are indicated, for safey we check the Prescription Drug Monitoring Program (PDMP) site each visit to ensure no other medications are being prescribed that are contraindicated with benzodiazepines.  When prescribing this class,  it is on a case-by-case basis and it is done very conservatively. 


At this time, patients new to Catherine will need to provide records detailing  past treatment for ADHD, a medication contract will need to signed, and a negative urine drug screen will be required for continuation of ADHD medication.  When prescribed, stimulants will only be prescribed in recommended doses (if a patient comes on higher than recommended doses, decreases will be discussed). If you are an adult with no record of treatment for ADHD, you will require formal psychological testing for ADHD confirmation prior  to initiation of treatment.


Due to delivering care virtually, we do not offer LAI prescription/maintenance at this time.
